'Memento' is the imperative remember! while 'nomen' means name.
The rest of the sentence is as follows: 'rogabam etiam Patrem Coelestem, ut quotiescumque nominaremus, nomen filii tui, id est, Iesu' (meaning I also was asking our Heavenly Father, so that however often we would call upon the name of his son, that is Jesus...) The imperative 'memento' doesn't make sense in the context. Since the name of Jesus follows, 'nomen' makes more sense.
